Thank you for choosing us as your dental care provider. Our office is committed to providing you with the best possible care.

Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. For the convenience of our patients, we accept Visa, MasterCard, Discover and American Express, as well as offer financing options through CareCredit. Payment for services is due at the time services are rendered.

Please ask a member of our staff, and we will review our payment options to accommodate your financial needs. If special arrangements are needed, please talk to our office manager prior to receiving service.

In-House Savings Plan

One of the goals of Alliance Dental is to provide quality dental care at an affordable price. However, the cost of health and dental care continues to increase each year. In an effort to encourage good dental health and alleviate rising dental costs, we have developed an "In-House" Dental Savings Program available to patients who cannot obtain dental insurance or find that our plan costs less than their current dental coverage plan. Since nearly all dental insurance plans require group participation to get any type of price reduction, individuals and families seeking dental insurance on their own often find it too expensive to even consider. Our dental program provides a wide scope of benefits at an affordable price to help minimize the cost of your dental needs.

Program Details:

Adult Program: $350

Ages 18 years and older
($306 Savings)

The Adult Program includes:

Comprehensive Exam (1 per year)
Panoramic X-Ray (Every 5 years)
Routine Cleanings (2 per year)
Bitewing X-Ray (2 per year)
Adult Periodic Exams (1 per year)
15% off all other dental procedures*

Adult Periodontal Program $500

($226 Savings)

The Periodontal Program includes:

Comprehensive Exam (1 per year)
Full Mouth X-Ray Series (1 every 5 years)
Comprehensive Periodontal Charting
FM Scaling and Root Planing (Once per year)
Periodontal Maintenance (2 per year)
Bitewing X-Ray (2 per year)
15% off all other dental procedures*

Children's Program: $250

Ages 3 years - 17 years
($225 Savings)

The Children’s Program includes:

Comprehensive Exam (1 per year)
Routine Cleanings (2 per year)
Bitewing X-Ray (2 per year)
Child Periodic Exams (1 per year)
Fluoride Treatment (2 per year)
15% off all other dental procedures*

No Waiting Period or Eligibility Periods for Major Treatment
No Missing Tooth Clause Restrictions or Exclusions
Annual Enrollment Fee for each family member is non-refundable.
Annual Enrollment Fees are due at the first appointment of the year or can be paid ahead early for the following year's enrollment. No exceptions.
Enrollment Date begins on the date the Enrollment fee is paid and shall expire after 365 days.
Family members can enroll at different times of the year. However, the end of the Coverage Year shall remain the same for all members, namely based on the Expiration Date of the first family member to enroll. To maximize coverage, all family members should enroll during the same month.
All treatments must be PAID IN FULL at each visit to keep the plan in effect.
The exams, bitewing X-rays, and cleanings must occur within the year of enrollment and cannot be carried over to the next year.
It is the patient's/parent's responsibility to make and keep appointments for his/her family members.
A $50 fee will incur for each broken appointment without a 24 hour advance notice.
Our program is not transferrable to another party or uncovered family member.
Participation cannot be combined with any other offer or dental plan.
All fees shall be based on practice's UCR fees and not any other dental plan's fees.
To be eligible for regular cleanings, patients must not have been diagnosed with Periodontal Disease. Patients with a history of Periodontic Disease must enroll in the Periodontal Program.
Annual Enrollment Fees and Procedures are subject to changes during the year.
Portability - You are not tied to any one employer. This is your plan and it follows you regardless of who you or your spouse work for during the year.

This is NOT dental insurance. Enrolling in our In-House Dental Savings Plan gives you the opportunity to obtain your dental treatment at reduced prices. It cannot be combined with dental insurance or other reduced fee dental plans.

VIP Program

This program was created for our Very Important Patients who are making a substantial contribution to our practice by referring friends, business colleagues, and non-immediate family members. These patients are very important to our continued growth and we would like to reward them with an even larger discount off their dental care during the year. This VIP Discount can be added to any of our Savings Programs and can be carried over for one additional year as our thanks for helping us grow our practice. To continue getting this VIP Discount, additional referrals are required each year.

VIP Silver

The VIP Silver Program involves an additional 5% Discount off all dental care during the enrollment year for referring 5 new patients who enroll in any Saving Program with any of our five offices. (Either 5 adults or a family of five)

VIP Gold

The VIP Gold Program involves an additional 8% Discount off all dental care during the enrollment year and the following year for referring 10 new patients who enroll in any Savings Program with any of our five offices. (Either 10 adults or various families totalling ten new patients or more)

VIP Platinum

The VIP Platinum Program involves an additional 15% Discount off all dental care during the enrollment year and the following year for referring 20 new patients who enroll in any Savings Program with any of our five offices. This will include small business owners who encourage their employees to enroll in one of our programs. This discount will continue for every year there are at least 20 employees and their immediate family members enrolled in one of our Savings Programs.
